Assisting Diabetes and Kidney Disease Communities

Diabetes affects millions of people worldwide. In some cases, it might progress to chronic kidney disease (CKD) and end-stage kidney disease (ESKD). The healthcare community is coming up with cuttingedge innovations to efficiently treat these conditions and improve diabetes and kidney disease outcomes.

Simcart is aiding the healthcare community in these efforts through its meticulously designed apps, RenalReality and Diabetes Reality, which help patients manage chronic kidney disease and diabetes. The apps leverage data aggregation technology to create automated care plans, enabling patients to take responsibility for their care.

“We created a user interface that assists the diabetes community, helping patients manage their condition from their first contact with medical services through all types of diabetes stages, including conservative care,” says Allister Williams, CEO of Simcart.

Simcart’s apps automatically input data using lab-generated medical information. For instance, when a patient undergoes a medical test, the data is automatically populated to a parallel database controlled and maintained by the Simcart team, whether for diabetes, renal function, or urine analysis. The data is examined by algorithms, which create color-indicated alerts, action plans, and action points based on the severity of the patient’s condition.
These relevant insights are visualized on the app’s dashboards, accessible for clinicians and patients after they complete the hospital-based presentation. Based on these insights, it crafts and delivers an algorithm-driven treatment plan to end users, whether the doctor or the patient. They get access to the actual action plan that is developed in less than 20 minutes.

The care plan is given to the general practitioner (GP), while keeping the patient in the loop. Simcart lets patients take responsibility for their renal or diabetic condition. It utilizes IT tools to interconnect four elements; awareness, screening for disease, managing disease, and patient ownership of the disease.

Patients can see the specialist within two months of getting a kidney, diabetes, or cholesterol test. If or when the patient falls in the red category, the Simcart team can pick them up and address the situation in advance.

Diabetes is associated with 40 percent of kidney patients, and there is a substantial gap in the market where kidney failure patients are not properly treated when they have early-onset diabetes. Simcart aims to fill this gap with Diabetes Reality, which went live on 27th November, 2022 and serves two distinct markets—diabetes and kidney disease—with about 40 percent overlap. Diabetes Reality expands Simcart’s capabilities to include the patient’s blood sugar levels and markers of diabetic disease.

We created a user interface that assists the diabetes community, helping patients manage their condition from their first contact with medical services through all types of diabetes stages, including conservative care.

The biggest patient challenge, complexity of data, is also solved by Simcart. It helps patients understand all the information shared by medical experts. Although clinician and patient interaction takes less than 20 minutes, its impact lasts years.

Simcart demonstrated its competencies in an instance where it identified and engaged patients before they or their physician was aware of the problem. It made early contact with a patient to address the issue and cost-effectively managed them.

Each Simcart application is designed by Williams, who has extensive experience as a clinician and programmer. When working as a kidney specialist, he realized the need for software solutions to assist the daily work of specialists and support the care journey of patients. His work has paved the way for software solutions that are changing the life of physicians and patients.

Moving forward, Simcart hopes to incorporate more intuitive features and push the boundaries of its solutions further to become a onestop shop for all diabetic and renal conditions.
